We’ve partnered with ChartSwap, a secure, HIPAA-compliant, web-based portal that allows for the electronic transfer of medical records and bills. This partnership ensures faster, more efficient processing and delivery of documents.
How to Submit a Request
- Prepare Your Release Form
Ensure you have a properly signed HIPAA-compliant medical release. - Submit Your Request
Upload your request electronically via ChartSwap. If additional documentation, such as an affidavit or certification, is required, simply specify your needs through the portal. - Choose Your Delivery Method
Select electronic delivery for faster processing. Note: EMS|MC is not responsible for documents lost in the mail if this method is chosen.
Payment Made Simple
- After submission, you’ll receive a confirmation email with a reference number.
- ChartSwap will invoice you electronically based on the complexity of the request and applicable state laws.
- Pay online via credit card or e-check through the ChartSwap portal.
Once payment is confirmed, ChartSwap will notify you when your documents are ready for download.
